Responsibilities

• Develop and implement individualized care plans with clients, families, and healthcare providers.
• Oversee finances: receipts, budgeting, and expenditures with proper documentation.
• Maintain open communication with clients, families, and healthcare professionals for satisfaction and continuity of care.
• Address client and caregiver concerns promptly and ensure service continuity.
• Manage administrative duties including budgets, schedules, and regulatory compliance.
• Foster community outreach and relationships with local healthcare providers and organizations.
