Job summary

FreedomCare LLC seeks a Licensed Practical Nurse Care Manager to support in-home services in Missouri. This field-based role requires frequent travel to patients’ homes across the Greater St. Louis area, conducting initial assessments, ongoing care coordination, and patient education.

You will collaborate with physicians and interdisciplinary teams to update care plans, ensure safety, and maintain regulatory compliance while delivering high-quality, person-centered care.

Qualifications

• Demonstrated commitment to high-level customer service with empathy and professionalism.
• Ability to follow scripts, workflows, policies, and regulatory requirements.
• Strong active listening with ability to understand needs and escalate when appropriate.
• Proficient with CRM systems and basic computer/touchscreen workflows.

Responsibilities

• Conduct initial in-person clinical assessments and coordinate timely service activation.
• Identify changes to plans of care and document revisions as needs change.
• Educate patients and caregivers on care needs, safety, and service requirements.
• Coordinate patient care through timely communication with physicians and internal teams.
• Provide ongoing education and coaching to support best practices and caregiver competency.
• Review service records and respond to changes in patient condition or needs.
• Identify safety concerns or urgent issues and escalate per protocols.
• Document assessments, visits, and follow-ups accurately within timeframes.
• Support authorization, renewal, start-of-care, and continued-service processes.
• Maintain regulatory compliance and company policies.
