Job summary

Ennoble Care is seeking a full-time Care Coordinator, Embedded to work at a senior living facility in Wayne, NJ. This role focuses on coordinating high-quality home-based care, collaborating with clinicians, staff and families to meet care goals while maintaining clear communication and organized support for caregivers.

The ideal candidate is compassionate, highly organized, and comfortable using Outlook and MS Office.

Qualifications

• Must be comfortable with speaking on the phone for large portions of the day.
• Compassionate and empathetic toward patients, with exceptional customer service.
• Able to take accurate notes and document tasks promptly.
• Able to multitask across patients and workstreams while staying organized and efficient.
• Willing to learn new skills continuously and adapt to change.

Responsibilities

• Complete individualized patient care plans and perform care management and coordination using the EMR system.
• Frequent contact with patients to provide care coordination and support, ensuring program compliance.
• Document all client communications accurately (verbal or written).
• Coordinate with Primary Care Clinician or designee on emergent needs and life-threatening events.
• Keep supervisor informed of issues related to care plans and any known issues.
• Collaborate across cross-organizational teams to meet patients' needs and family goals.
• Become proficient with technology including secure email, phone systems, and EMR.
• Adhere to documentation protocols and maintain daily logs and internal communications.
• Provide excellent customer service and contribute positively to the team.
