Job summary

Ennoble Care in Marlton, New Jersey is seeking a full-time Patient Care Coordinator to join our home-based and clinic team. You will work with clinicians, staff, and patients to coordinate care, schedule providers, and support families, ensuring high-quality care.

The role requires compassionate, organized individuals who can manage multiple patients and communicate clearly using Outlook and our EMR system. Monday to Friday, 8:30am-5:00pm on-site; pay starts at $18 per hour with strong benefits.

Qualifications

• Must be comfortable with speaking on the phone for large portions of the day.
• Compassionate and empathetic towards patients with strong customer service.
• Able to take accurate notes and document tasks promptly.
• Capable of multitasking between patients and workstreams while staying organized.
• Thrives in a fast-paced environment; on-site Marlton, NJ schedule.

Responsibilities

• Complete individualized patient care plans and perform care management and coordination using the EMR.
• Maintain frequent contact with patients to provide care coordination and support to improve outcomes.
• Document all client communications accurately.
• Coordinate with Primary Care Clinician regarding emergent needs and ensure care plans are complete.
• Inform Team Supervisor of issues related to the care plan process.
• Collaborate with cross-organizational areas to meet patient needs and family expectations.
• Become proficient with technology including secure email, phone system, EMR, etc.
• Adhere to documentation protocols and daily work logs, escalation, and internal communications.
• Provide excellent customer service evidenced by positive feedback.
• Support department teammates to contribute positively and constructively.
