Ennoble Care is seeking a full-time Care Coordinator to work at a client's senior living facility in Hillsborough, NJ. You will coordinate patient care plans, communicate with clinicians, and support families to achieve optimal healthcare outcomes.

The role emphasizes compassionate service, accurate documentation, and teamwork in a fast-paced environment. Excellent organizational skills and proficiency with Outlook and MS Office are required.

Qualifications

• Must be comfortable speaking on the phone for long periods.
• Compassionate and empathetic towards patients with exceptional customer service.
• Ability to take accurate notes and document tasks promptly.
• Multitask across patients and workstreams while staying organized.

Responsibilities

• Create individualized patient care plans and perform care coordination using Ennoble Care’s EMR.
• Maintain frequent contact with patients to support care coordination and program compliance.
• Document all client communications accurately.
• Communicate with Primary Care Clinician regarding emergent needs to ensure comprehensive care plans.
• Keep Team Supervisor informed of issues related to care plans.
