Ennoble Care is seeking a full-time Care Coordinator, Embedded to work in a client’s senior living facility in Wayne, NJ. You will ensure high-quality care, collaborate with clinicians, staff, and families, and manage caregivers to meet care goals.

The role focuses on care management, documentation, and strong communication to support patient outcomes. In-office daytime schedule, Monday–Friday, with competitive benefits and a pay rate of $19.00 per hour.

Qualifications

• Must be comfortable speaking on the phone for long periods.
• Compassionate and empathetic toward patients with excellent customer service.
• Ability to take accurate notes and document tasks promptly.
• Ability to multitask across patients and workstreams while staying organized.
• Thrives in a fast-paced environment.
• Full-time, Monday–Friday, daytime hours, in Wayne, NJ.
• Proficient with computers, including Outlook and Office programs.
• Knowledge of basic healthcare terms, conditions, roles, and care principles.
• Willing to learn new skills continuously and be a team player.

Responsibilities

• Develop and manage individualized patient care plans and care coordination using Ennoble Care’s EMR system.
• Maintain frequent contact with patients to provide support and ensure compliance with care management programs.
• Document all client communications accurately.
• Communicate with Primary Care Clinician or designee about patient needs and care plans.
• Inform Team Supervisor of issues related to the care plan process.
• Collaborate with cross-organizational teams to meet patient needs and coordinate with families.
• Learn to use technology such as secure email, phone system, EMR, etc.
