Visiting Angels of Fairfield seeks a dedicated Full-Time Client Service Coordinator to supervise in-home care for seniors. You will assess daily needs, develop care plans, and schedule caregivers while maintaining strong client and family relationships.

Ideal candidates have a college degree, experience with seniors, and excellent written and verbal communication skills. Fast-paced team environment requires problem-solving, discretion, and independent decision-making.

Qualifications

• Candidates must have a college degree and experience working with seniors in home or facility settings.
• Strong written and verbal communication skills are required.
• Ability to multi-task and manage multiple disruptions in a fast-paced environment.
• Excellent problem-solving, discretion, and independent decision-making abilities.

Responsibilities

• Process referrals from intake via email or phone in a timely manner.
• Match clients with suitable caregivers to meet care needs.
• Ensure cases are staffed appropriately and coverage is arranged.
• Coordinate on-call coverage for evenings and weekends on a rotating schedule.
• Maintain client and caregiver records in the Wellsky system and communicate concerns promptly.
• Build relationships with clients and family members and provide exceptional support.
