Qualifications

• Degree or associate degree in a health care or related field or social work and two years of supervisory experience in homecare ideal
• Clinical licenses such as LPN or CNA preferred but not required
• Experience in home care, medical office administration, or social services with home visits preferred
• Proficiency in Microsoft Office required
• Ability to plan, organize, prioritize and meet deadlines
• Ability to stay calm, flexible and maintain humor in all situations
• Professional appearance reflecting business standards
• Clear communication in writing and speaking
• Ability to work independently with minimal supervision
• Strong interpersonal and problem-solving skills
• Excellent phone etiquette and customer service with multitasking
• Knowledge of healthcare software such as WellSky and ClearCare a plus
• Sales or marketing background a plus
• Legal ability to work in the U.S.

Responsibilities

• Serve as the primary point of contact for client communications
• Manage client care needs including scheduling initial assessments and developing a care plan
• Document and update client and caregiver records in Clear Care
• Process and track clients’ and caregivers’ leads in lead systems
• Collect client agreements and review for completeness and communicate with clients
• Support on-call coverage and resolve issues
• Schedule caregivers and cover fill-ins and emergencies
• Hire, onboard and evaluate caregivers to reduce turnover
• Collaborate with scheduler and marketer to match caregivers to clients
• Develop Plans of Care and update as needed
• Conduct supervisory visits to ensure services follow the Plan of Care
• Perform client reassessments quarterly or as needed
• Resolve complaints and monitor client and employee satisfaction
• Maintain confidentiality of employee and client information
• Maintain professionalism and respond calmly in emergencies
• Demonstrate time management and organization, follow instructions
• Show self-motivation, initiative and ability to coordinate multiple tasks
• Agree to be on on-call rotation
• Build relationships with clients, families and caregivers
• Comply with laws and regulations
• Provide feedback to management and demonstrate commitment to client service
• Continuously strive for service excellence
