Zusammenfassung

Transcend-Hospice seeks a caring aide to provide direct patient care in patients' homes under the supervision of a registered nurse. You will assist with transfers, document care, and support infection control as part of a compassionate hospice team.

The role requires a high school diploma or approved training, a valid driver's license, and the ability to read and write in English. Experience and state-approved certifications are preferred, with flexibility for local travel.

Qualifikationen

• High School Diploma preferred; options include Medicare §418.76 variants or state-approved competency eval programs.
• Nurse Aide training/competency eval program approved by the state or state licensure program as per §418.76(b)(c).
• Must have current Driver’s License.
• At least one year experience as hospice aide/NCA in relevant settings or completion of CNAs program.

Aufgaben

• Provide direct patient care to patient under direction of the RN and according to the Aide Plan of Care.
• Assist with safe transfers and ambulation per agency policy.
• Report changes and document pertinent information and care rendered to ensure continuity of care.
• Practice infection control and maintain a clean, safe environment.
• Communicate with patient/family respectfully; collaborate with staff.
• Respect privacy and property and promote supportive care.
• Support proper nutrition and adequate fluid intake for patients.
• Demonstrate time management skills and contribute to agency operations.
• Assist with treatments and procedures per policy while ensuring quality of care.
