Job summary

Transcend Hospice seeks a Hospice Aide to provide compassionate, in-home care under the supervision of a registered nurse. You will assist with transfers, daily living activities, and observation reporting to ensure quality end-of-life care for patients and families.

Requirements include a high school diploma; completion of a Nurse Aide program or state-approved competency evaluation; current driver’s license; and strong English reading/writing skills.

Qualifications

• Reads and writes in English; follows English instructions.
• Provide direct patient care per RN direction and Plan of Care.
• Assist with transfers, ambulation and safe mobility per policy.

Responsibilities

• Provide direct patient care to patient under direction of the RN.
• Assist with safe transfers and ambulation.
• Report changes and document care rendered for continuity of care.
• Maintain infection control and a clean, safe environment.
• Communicate respectfully with patient/family and employees.
• Respect patient privacy and property.
• Support proper nutrition and fluid intake.
• Contribute to agency operations and time management.
• Perform treatments and procedures per policy.
