Job summary

Transcend Hospice is seeking a Hospice Aide to provide in-home healthcare services under the supervision of a registered nurse. The role focuses on safety, comfort, and compassionate support for patients and families.

Responsibilities include direct patient care, safe transfers, reporting changes, infection control, and ensuring a clean, respectful home environment. A valid driver's license and English proficiency are required.

Qualifications

• High School Diploma preferred; CNA or Nursing Assistant program or state licensure may meet requirements.
• Must be able to read and write in English and follow written and verbal instructions.
• Must have a valid driver’s license.

Responsibilities

• Provide direct patient care to patients under RN direction.
• Assist with safe transfers and ambulation per policy.
• Report changes and document care to ensure continuity of care.
• Maintain infection control and provide a clean, safe environment.
• Communicate respectfully with patient/family and staff.
• Respect patient privacy and property.
• Assist with nutrition and fluid intake safely.
• Support agency efficiency with time management.
• Perform treatments and procedures per policy.
• Promote agency philosophy and policies to ensure quality care.
