Job summary

Transcend-Hospice seeks a compassionate Hospice Aide to deliver direct patient care under the supervision of a registered nurse in patients' homes. You will assist with transfers, provide basic care, and document observations to support continuity of care.

The role requires English literacy, a current driver’s license, and CNA or approved training with experience preferred in hospice or long-term care settings. Travel to local home visits is expected.

Qualifications

• High School Diploma preferred; completion of Medicare §418.76 (b)(c) programs or equivalent.
• Must have current Driver's License.
• At least one year of hospice aide or CNA experience preferred; can include CNA program completion.

Responsibilities

• Provide direct patient care under the direction of an RN.
• Assist with safe transfers and ambulation per policy.
• Report changes and document care to ensure continuity of care.
• Maintain infection control and a clean, safe home environment.
• Communicate respectfully with patient, family, and staff.
• Travel to patient homes and adapt to various home settings.
