Job summary

Providence at Home with Compassus seeks a Home-Based Acute Care Occupational Therapist to deliver inpatient-level therapy in the home, conducting evaluations and creating personalized treatment plans. You’ll work independently within an interdisciplinary team to support critical transitions and promote independence in a groundbreaking care model.

The role requires OT licensure, BLS within 30 days, and a preference for 1 year of home health experience with adults.

Qualifications

• Bachelor’s degree in Occupational Therapy or Master’s degree in Occupational Therapy.
• Current OT license in good standing (State of Employment).
• BLS certification within 30 days of hire.
• 1 year OT experience with adults in home health (preferred).

Responsibilities

• Deliver comprehensive therapy services including assessment, intervention, care planning, implementation, and discharge planning.
• Evaluate clients' physical, cognitive, psychological, and perceptual functions for safe home function.
• Develop individualized plans of care with patients and families.
• Supervise and instruct Home Health Aides and COTAs as directed.
