Providence at Home with Compassus is seeking a Home-Based Acute Care Occupational Therapist to bring inpatient-level therapy into patients' homes. You will perform comprehensive evaluations, create personalized treatment plans, and guide safe transitions from hospital to home.

Qualifikationen

• Current, unencumbered OT license in the state of employment.
• BLS certification within 30 days of hire.
• Bachelor’s or Master’s degree in Occupational Therapy.

Aufgaben

• Deliver comprehensive therapy services including assessment, intervention, care planning, implementation, and discharge planning.
• Evaluate clients’ physical, cognitive, psychological, and perceptual functions for home safety.
• Develop individualized plans of care in collaboration with patients and families.
• Supervise Home Health Aides and provide case management for clients with COTAs.
• Document the same day as visits and participate in team meetings.
