Job summary

Providence at Home with Compassus is hiring for a Home-Based Acute Care Occupational Therapist. You will deliver in-home, inpatient-level therapy, perform evaluations, create treatment plans, and ensure safe discharge planning as part of a joint venture between Providence and Compassus.

You will work independently within a multidisciplinary team, manage a caseload, supervise aides, and contribute to staff onboarding and continuous care improvements in a home health setting.

Qualifications

• Bachelor's degree in Occupational Therapy or Master's degree in Occupational Therapy.
• Current, unencumbered OT license in the state of employment (within 30 days of hire).
• BLS certification (American Heart Association).

Responsibilities

• Deliver comprehensive home-based occupational therapy services including assessment, treatment planning, and discharge planning.
• Evaluate physical, cognitive, and perceptual functions to ensure safe home functioning.
• Develop individualized plans of care with patients and families.
• Supervise and educate Home Health Aides and COTAs.
• Complete documentation the same day as visits and participate in team meetings.
