Job summary

Providence at Home with Compassus seeks a Home-Based Acute Care Occupational Therapist to bring inpatient-level therapy into patients’ homes. You will evaluate, treat, and discharge planning while working independently within a multidisciplinary team to restore independence.

The role emphasizes in-home evaluations, personalized treatment plans, patient and family education, and seamless coordination with caregivers. Licensed OT with current BLS is required; 1 year in home care preferred.

Qualifications

• Required - Bachelor's or Master's degree in Occupational Therapy.
• Preferred - 1 year of Occupational Therapy experience in home care.

Responsibilities

• Deliver comprehensive therapy services in the home, including assessment, intervention, care planning, implementation, and discharge planning.
• Evaluate clients’ physical, cognitive, psychological, and perceptual functions for safe home functioning.
• Develop individualized plans of care with patients and families.
• Supervise Home Health Aides and provide case management for clients under COTAs.
• Document on the day of visits per guidelines and assist onboarding of new team members.
