Job summary

Conduit Health in New York is seeking an Implementation Manager to optimize state launches, payor implementations, and cross-functional initiatives. This role sits on the DME Operations team and connects Clinical Ops, Support, Payor Development, and Product to land new initiatives.

You’ll drive launch plans, track deliverables, and ensure timely execution with a focus on patient outcomes in a fast-growing startup environment.

Responsibilities

• Own new state expansions from post-diligence handoff through go-live, building launch plans and punch lists.
• Lead payor implementations and launches, coordinating eligibility, credentialing, billing, and fulfillment readiness.
• Act as connective tissue across teams, chasing commitments and unblocking handoffs to keep launches moving.
• Build and own source-of-truth launch trackers to show what’s done, next, and ownership.
• Run kick-offs, surface risks early, and course-correct before a dependency slips.
• Potentially own network contracting admin for land-and-expand model.
• Report against launch KPIs and translate learnings into repeatable playbooks.
