The position focuses on targeted case management, treatment planning, counseling, and assistance with benefits, housing subsidies, and landlord coordination to help clients secure and maintain housing.

Qualifications

• Bachelor’s Degree in human services or equivalent education and experience.
• 2 years experience working with the dually diagnosed (mentally ill, substance abuse).
• Valid driver’s license. CPR/First-Aid Certification within 6 months of hire desired.

Responsibilities

• Provide targeted case management such as establishing rapport with clients, writing comprehensive treatment plans, provides counseling and support, providing direct skill teaching, advocacy and assistance in obtaining/maintaining benefits, budgeting, treatment, medical services, employment, transportation etc.
