Job summary

Community Mental Health Affiliates, Inc is hiring a full-time Case Manager for the Community Support Program in New Britain, CT. The role involves direct client care, education, and coordination of services to support recovery from mental illness and co-occurring conditions.

You will develop and review individualized treatment plans, document progress, and collaborate with internal teams to ensure comprehensive care.

Qualifications

• Bachelor’s degree preferred or equivalent experience.
• Experience in case management and understanding mental health and co-occurring conditions.
• Proficiency with Microsoft Office and EMR systems.

Responsibilities

• Maintain caseload as determined by CMHA and funder.
• Provide direct care with education, skill building, recreation, and environmental monitoring.
• Teach life skills to help clients live independently.
• Develop and review individualized treatment plans and document progress.
• Ensure timely documentation per CMHA policies and procedures.
• Supervise self-administration of medication if applicable and compliant.
• Collaborate with CMHA teams and coordinate with external providers.
• May provide transportation as needed.
