Job summary

Lighthouse Academies seeks two Supportive Housing Case Managers to serve formerly homeless and at-risk individuals and families in the Southfield area, operating with Housing First principles.

The roles involve coordinating housing solutions, maintaining client caseloads, and connecting clients with essential resources while collaborating with onsite service providers to promote stability and sobriety.

Qualifications

• Bachelor's degree or five years of experience in human/social services or case management.
• Experience working with people experiencing homelessness or extreme poverty in the US.
• Strong written and verbal communication skills and attention to detail.

Responsibilities

• Screen applications and connect eligible individuals to Permanent Supportive Housing opportunities.
• Maintain a caseload of clients (approx. 15) and provide wrap-around services.
• Outreach to landlords for mediation and tenant relations.
• Connect clients to resources for move-in costs, furniture, and moving services.
• Provide Housing First case management support and timely program entries in HMIS.
• Coordinate with property managers and assist with monthly housing inspections.
