Job summary

Lighthouse MI is seeking two Supportive Housing Case Managers in Southfield, MI to assist formerly homeless and at-risk households with securing long-term housing solutions. You will provide wrap-around services, connect clients to community resources, and work closely with on-site teams to support housing stability.

Ideal candidates bring trauma-informed care experience, strong de-escalation skills, and the ability to navigate HMIS and housing programs.

Qualifications

• Experience with homelessness or extreme poverty in the US is required.
• 1 year of experience in human/social services and/or case management.
• Bachelor's degree or five years of related experience.
• Reliable transportation and auto insurance.
• Valid driver's license; driving record check may be required.
• Proficiency with Microsoft Office Suite and Google Apps.
• Ability to prioritize tasks in a fast-changing environment.
• Ability to multitask in a fast-paced setting.
• Ability to work with a diverse population.

Responsibilities

• Screen applications and connect eligible community members to housing opportunities.
• Screen for MSHDA Section 8 vouchers and manage waitlists/portals as applicable.
• Maintain a caseload of 15 clients.
• Assist clients in obtaining personal documents for housing programs.
• Outreach to landlords for mediation and tenant relations.
• Link clients to move-in cost resources and services.
• Provide ongoing case management using Housing First with monthly in-home/in-community support.
• Enter program data and case notes in HMIS within 48 hours when possible.
• Collaborate with Property Managers and conduct monthly housing inspections.
• Support with general duties and crisis response as needed.
