Job summary

Molina Healthcare is seeking an RN care manager in Naperville, IL to support LTSS coordination and care across the continuum. You will collaborate with a multidisciplinary team to ensure progress toward outcomes and cost-effective member care.

Responsibilities include conducting assessments, developing care plans, monitoring outcomes, and coordinating waiver services with members, caregivers, physicians and the care network. Travel may be required.

Qualifications

• At least 2 years of experience in health care, including at least 1 year in care management or LTSS.
• RN license must be active and unrestricted in state of practice.
• Bachelor's degree in a health care related field may be required in some states.
• Valid driver's license and reliable transportation for travel requirements.
• Ability to operate proactively and manage multiple priorities.

Responsibilities

• Complete comprehensive member assessments within regulated timelines, including in-person home visits as required.
• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members and care teams.
• Monitor care plans and adjust interventions as needed.
• Promote integration of behavioral health and LTSS and community resources.
• Assess medical necessity and authorize waiver services.
