• Facilitate waiver enrollment/disenrollment processes.
• Develop and implement care plans with members, caregivers and clinicians.
• Monitor care plans for effectiveness and adjust as needed.
• Coordinate LTSS, behavioral health and community resources.
• Identify barriers to care and address psycho/social, financial and medical obstacles.
• Review benefits and funding sources for appropriate services.
