Job summary

Molina Healthcare is seeking an experienced Registered Nurse (RN) to support care management and LTSS coordination in Illinois. You will work with a multidisciplinary team to plan, implement, and monitor care plans for high-need members to improve outcomes while managing costs.

Responsibilities include comprehensive assessments, waiver enrollment, care plan development, and proactive support for barriers to care. Travel may be required.

Qualifications

• RN license active and unrestricted.
• 1+ year care management, managed care, or LTSS experience.
• Ability to travel 25-40% locally as required.

Responsibilities

• Completes member assessments within regulated timelines, including home visits.
• Develops care plans and coordinates waiver services with care team.
• Monitors care plan effectiveness and documents progress.
• Educates members and families and facilitates ICT meetings.
• Assesses barriers to care and supports members with psycho/social needs.
