Job summary

Molina Healthcare is seeking a Registered Nurse to join our care management team in Illinois. You will coordinate LTSS, conduct assessments within timelines, develop and monitor care plans, and work with the interdisciplinary team to improve member outcomes.

You will travel locally (25–40%) and use motivational interviewing, maintain compliance, and collaborate with care teams and caregivers to ensure quality and cost-effective member care.

Qualifications

• Active RN license in state of practice.
• 2+ years in health care, with ≥1 year in care management or LTSS.
• Experience with disability, chronic conditions, substance use, and LTSS preferred.

Responsibilities

• Complete member assessments within regulated timelines and during home visits.
• Develop and implement care plans including waiver service plans with the care team.
• Monitor care plan effectiveness and document interventions and outcomes.
• Coordinate care with interdisciplinary team to ensure quality and cost-effective care.
• Travel locally 25–40% as required by state contracts.
