Job summary

Geisinger Wyoming Valley (GWV) is hiring a Registered Nurse (RN) to join the Care Coordination team. The role is full-time, exempt, with a Monday–Friday daytime schedule and weekend rotation roughly every third weekend. A PA RN license is required and at least 3 years of related experience is preferred.

The position emphasizes evidence-based care, collaborative planning, and ongoing evaluation of patient health status to improve outcomes within Geisinger’s patient-centered care model.

Qualifications

• Graduate from Specialty Training Program-Nursing is required.
• Bachelor’s Degree in Nursing is preferred.
• RN license in Pennsylvania is required.

Responsibilities

• Assesses, plans, implements, coordinates, monitors and evaluates care to optimize patient health.
• Develops individualized plans of care with a team approach.
• Maintains documentation for case management activities.
