Job summary

Geisinger Health Plan is seeking an RN Case Manager to coordinate patient care from home in Pennsylvania. You’ll join a collaborative team to advance our vision of making better health easier for our community, using your nursing expertise to connect members with providers and services.

This exempt, full-time role requires at least 2 years RN experience; ACMA CCM preferred and BSN preferred. You’ll work days, 40 hours weekly, with a comprehensive benefits package starting day one.

Qualifications

• RN work experience: at least 2 years required.
• ACMA CCM certification preferred.
• BSN preferred.
• Case management experience preferred.
• Knowledge of healthcare laws, regulations, and insurance policies is helpful.
• Ability to work autonomously and in a multidisciplinary team is helpful.

Responsibilities

• Assess, plan, implement, coordinate, monitor and evaluate care to optimize health status.
• Develop care plans aligned with guidelines and protocols.
• Monitor progress and promote early intervention in acute care situations.
• Collaborate with patient, family, and care team for coordinated services.
• Maintain documentation for all case management activities.
