Geisinger Wyoming Valley (GWV) Care Coordination is seeking an experienced Registered Nurse to join a full-time, exempt team. The role involves coordinating care, planning and evaluating services to optimize patient health, with a Monday–Friday schedule and weekend rotation.

Three years of RN work experience is required. Comprehensive benefits and a supportive work environment are offered.

Qualifications

• At least three (3) years of RN work experience required.
• Clinical coordination and care management responsibilities expected.

Responsabilités

• Assesses, plans, implements, coordinates, monitors and evaluates all options and services to optimize patient health status.
• Manages utilization and practice metrics to refine care delivery and outcomes.
• Integrates clinical guidelines and protocols to develop patient-centered treatment plans.
• Develops systems of care that monitor progress and promote early intervention in acute care situations.
• Collaborates with patient, family, primary care provider, and health care team to ensure coordinated services.
