Qualifications

• Graduate from Specialty Training Program-Nursing (Required).
• Minimum of 2 years nursing experience (Required).
• Licensed Registered Nurse (PA) and BLS certification required.

Responsibilities

• Facilitates entrance of new patients into care programs.
• Provides patient education and ongoing continuity of care.
• Coordinates pre- and post-hospital care and serves as liaison between patient and caregivers.
• Collaborates with multidisciplinary clinics and supports the provider's plan of care.
