Geisinger Wyoming Valley in Pennsylvania seeks an experienced Registered Nurse to join the Care Coordination team. You will assess, plan, implement, and monitor patient care, collaborating with providers to optimize health outcomes for the identified population.

The role emphasizes evidence-based practice and patient-centered care in a clinical setting. The schedule is full-time, days, with a rotating weekend and in-house holidays.

Qualifications

• Graduate from Specialty Training Program-Nursing (Required)
• Bachelor's Degree-Nursing (Preferred)

Responsabilités

• Join Care Coordination team supporting GWV.
• Typical schedule is full time, exempt, Monday through Friday (days).
• There is a weekend rotation (about every third weekend).
• In-house holiday rotation.
• At least three (3) years of RN work experience is required.
