Job summary

Geisinger Wyoming Valley (GWV) seeks an RN to join an interdisciplinary care team, promoting preventative patient care and coordinating disease management services. The role includes clinical support to providers, triage of patient communications, and education for patients and families within Care Coordination programs.

We value professional growth, evidence-based practice, and collaboration, with full-time hours Monday through Friday, 8:00 a.m.–4:30 p.m.

Qualifications

• Graduate from Specialty Training Program-Nursing (Required)
• Bachelor's Degree in Nursing (BSN) preferred
• Minimum of 2 years-Nursing experience (Required)
• Basic Life Support Certification and PA RN license

Responsibilities

• Provide primary clinical support and disease management in an interdisciplinary team.
• Triage in-basket messages and manage patient interactions.
• Coordinate labs and diagnostic tests; document in EPIC.
• Educate patients and families using Care Coordination programs.
