Job summary

Talbert House seeks a Residential Corrections Case Manager to provide structured case management to residents in a residential corrections setting. You’ll supervise caseloads, conduct interviews, assess needs, and develop individualized service plans in collaboration with a treatment team.

You will use core correctional practices to promote accountability and assist residents toward positive behavior changes and successful program completion.

Responsibilities

• Maintain an assigned caseload and provide structured case management services to residents.
• Conduct resident interviews and administer assessments under supervision.
• Assess needs, develop individualized service plans, and monitor progress toward goals.
• Lead educational groups and facilitate group curricula to support behavior change and reentry.
• Communicate effectively with corrections, clinical, administrative staff to support coordinated resident services.
• Maintain timely documentation in accordance with policies and regulations.
