Qualifications

Responsibilities

• Responsible for full client care cycle: assess, plan, implement, monitor, evaluate.
• Coordinate and provide safe, timely, effective, equitable, client-centered care.
• Maintain caseload and contact clients to monitor progress.
• Draft service plans, review progress, determine closure with growing independence.
• Deliver ongoing services including crisis intervention, advocacy, transportation, housing aid, referrals.
• Assist wellness through supportive counseling and care coordination.
• Develop and implement individualized client service plans and ensure compliance.
• Document clinical activities for program and funding criteria.
• Participate in treatment team; recommend client status changes.
• Lead educational groups or facilitations when required.
• May transport clients in vehicle as needed.
• Adhere to professional standards and agency protocols.
