Responsibilities

• Perform acuity assessments and income verification every 6 months.
• Develop and update ISPs based on acuity assessments.
• Assist with insurance navigation and benefits enrollment.
• Coordinate referrals to social services and housing programs.
• Maintain EMR documentation and contract data reporting.
• Collaborate with CID and BMC teams to promote care.
