Job summary

Boston Health Care for the Homeless Program (BHCHP) seeks a Manager of HIV Case Management and Support Services to lead a multidisciplinary team of medical, outreach, and housing case managers delivering comprehensive, patient-centered care to people living with HIV.

You will oversee case management operations, manage Ryan White Program resources, and collaborate across BHCHP to reduce barriers to care, promote housing stability, and address social determinants of health.

Qualifications

• Associate or bachelor’s degree in a related field; two years’ relevant human services experience.
• Two years of management experience.
• Experience working with people with substance use disorders.
• Knowledge of trauma-informed approaches and behavioral risk reduction.
• Bi-lingual fluency in Spanish preferred; experience with diverse populations.

Responsibilities

• Hire, train, supervise and evaluate HIV team case managers.
• Provide monthly supervision and weekly huddles.
• Oversee resource management and grant oversight.
• Coordinate integration with HIV program and BHCHP.
• Ensure quality assurance and program reporting.
• Direct medical case management and patient-centered care.
