Job summary

Boston Public Health Commission seeks an HIV Nurse Case Manager to deliver client-centered clinical case management for adolescents and adults living with HIV/AIDS. The role collaborates with the care team to ensure timely access to primary care, medications, and adherence support.

You will conduct comprehensive assessments, develop ISP and nursing care plans, coordinate referrals, educate patients, and maintain detailed documentation to support positive health outcomes.

Qualifications

• Bachelor’s degree preferred (BSN or equivalent)
• Massachusetts RN license required
• Current CPR certification
• Minimum 3 years nursing experience preferred; 2 years in acute/medical-surgical care
• HIV/AIDS knowledge or experience preferred

Responsibilities

• Provide client-centered clinical case management for HIV patients.
• Coordinate access to primary care, medications, and adherence support.
• Develop and monitor individualized service plans (ISP) and nursing care plans.
• Collaborate with care team; review labs, labs, and follow-ups.
• Document care activities and maintain records per policy.
