Job summary

Boston Health Care for the Homeless Program is seeking an HIV Nurse Case Manager to join a multidisciplinary HIV primary care team in greater Boston. The role spans outpatient clinics, shelters, street outreach, and patients’ homes, with a focus on coordinated, compassionate care.

You will manage HIV and chronic diseases, conduct triage, support medication adherence, and educate patients across diverse settings. Funding extends through March 2028 with potential renewal.

Qualifications

• Licensed as a Registered Nurse in Massachusetts (MA).
• Bachelor’s degree in nursing preferred; 2 years ambulatory/outpatient care experience required.
• ACRN certification preferred; funding available to obtain this after hire.

Responsibilities

• Provide outpatient primary care nursing and triage; document in EHR.
• Implement patient-centered care plans; conduct intake assessments.
• Administer vaccines and wound care; provide adherence counseling.
• Coordinate care with HIV team; visit shelters and outreach sites.
• Manage HIV and chronic diseases; identify barriers to care.
• Support follow-up for medications (buprenorphine/naltrexone) for OUD; provide preventive services.
• Promote compassionate care; train new nursing staff.
• Maintain professional relationships with care team and facilities.
