Positive Impact Health Centers, Inc. is seeking a Medical Case Manager to provide comprehensive case management to adult clinic patients within a team-based care model.

This role supports access to medications, adherence, and social services while ensuring compliance with HRSA/Ryan White guidelines. The MCM will work with an interdisciplinary team to develop individualized service plans, monitor progress, coordinate referrals, and document care activities in the EMR, all while maintaining HIPAA

Qualifications

• Social service or nursing degree from an accredited program.
• BSW or MSW preferred for advancement.
• Spanish fluency is highly preferred.

Responsibilities

• Learn and stay current on HIV/AIDS practice issues.
• Provide medical case management to identified patients per HRSA/Ryan White guidelines.
• Develop ISP for a caseload of ~70 patients and monitor progress.
• Coordinate referrals for medical and non-medical services.
• Document patient information accurately in the EMR.
• Maintain HIPAA confidentiality and compliance.
• Participate in an interdisciplinary care team to assess needs.
