Evolve Therapy Services is hiring a Physical Therapy Assistant for an in-house Skilled Nursing Facility in Warren, PA. This PRN role supports the Physical Therapist and provides therapy services to residents under supervision.

Responsibilities include screenings, treatment implementation, and documentation in NetHealth EMR, with focus on mobility, strength, balance, and functional goals. Ongoing licensure and continuing education are required.

Qualifications

• Associate's degree in physical therapy from an accredited program.
• Active Physical Therapy Assistant license in state(s) of practice.
• Commitment to ongoing continuing education and licensure maintenance.
• Maintain current licensure and certification.

Responsabilités

• Provide physical therapy services to residents under the direct supervision of the Physical Therapist.
• Conduct screenings of residents to determine need for intervention or treatment.
• Assist in evaluations and in developing treatment plans with the PT.
• Implement individual and group treatment per the treatment plan.
• Monitor residents' response and modify as directed by the PT.
• Assist in scheduling and coordinating resident therapies with the team.
• Document compliant and medically necessary CMS/NetHealth records.
• Educate residents, families, and staff on therapy procedures and safety.
