Job summary

Sunrise Community of Southwest Florida, Inc. is seeking a Direct Support professional to provide assistance to individuals with intellectual and developmental disabilities in the community.

The role emphasizes individual-centered plans, active participation, and meticulous documentation for regulatory compliance. The position requires a valid driver’s license, a high school diploma (Bachelor’s preferred), and background/drug screenings.

Qualifications

• Completion of pre-employment background and drug screenings.
• Valid driver’s license with endorsements and at least 3 years driving experience.
• Advanced computer skills.
• High school diploma or equivalent; bachelor’s degree preferred.
• Experience working in intellectual/developmental disabilities preferred.
• Minimum one year of writing program plans, case notes or other relevant documentation.

Responsibilities

• Develop, implement and coordinate individual’s program plans.
• Develop and monitor individualized training programs to meet priority training needs.
• Ensure all plans are person centered and reflect the individual’s goals through participation.
• Ensure documentation is accurate and complete per regulatory requirements.
• Communicate with family, guardians and support coordinators to address needs.
• Perform other duties as assigned.
