Job summary

Sunrise Community, Inc. seeks dedicated Direct Support professionals to assist individuals with intellectual and developmental disabilities in the community.

The role focuses on developing and coordinating individualized program plans and training while ensuring person-centered approaches and compliant documentation. The position involves collaboration with families, guardians, and support coordinators to meet each individual's needs.

Qualifications

• Must complete pre-employment Background and Drug Screenings.
• Must possess a valid Driver's License with endorsements and have at least 3 years of driving experience.
• Must possess advanced computer skills.
• Must possess a High School Diploma or equivalent, bachelor's degree preferred.
• Experience working in the field of intellectual/developmental disabilities preferred.
• Minimum one year of experience in writing program plans, case notes or other relevant documentation.

Responsibilities

• Develop, implement and coordinate individual's program plans.
• Develop, implement and monitor individualized training programs to meet identified priority training needs.
• Ensure all plans are person centered and reflect the individual's hopes and dreams through active participation wherever possible.
• Ensure all documentation is accurate and complete according to regulatory requirements.
• Communicate with all team members, including family, guardians and support coordinators to ensure that the individual's needs are being addressed.
• Performs other duties as assigned.
