Job summary

Sunrise Community, Inc. is seeking a compassionate Direct Support professional to help individuals with intellectual and developmental disabilities live valued lives in the community.

You will develop, implement, and monitor individualized program plans, and coordinate training to address identified needs. This role emphasizes person-centered planning, accurate documentation, and collaboration with families, guardians, and support coordinators.

Qualifications

• Must successfully complete the required pre-employment Background and Drug Screenings.
• Must possess a valid Driver's License with endorsements and have a minimum of 3 years of driving experience.
• Must possess advanced computer skills.
• Must possess a High School Diploma or equivalent, bachelor's degree preferred.
• Experience working in the field of intellectual/developmental disabilities preferred.
• Minimum one year of experience in writing program plans, case notes or other relevant documentation required.

Responsibilities

• Development, implementation and coordination of individual's program plans.
• Develop, implement and monitor individualized training programs to meet identified priority training needs.
• Ensure all plans are person centered and reflect the individual's hopes and dreams through active participation wherever possible.
• Ensure all documentation is accurate and complete according to regulatory requirements.
• Communicate with all team members, including family, guardians and support coordinators to ensure that the individual's needs are being addressed.
• Performs other duties as assigned.
