Job summary

Sunrise Community of Southwest Florida, Inc. is seeking a dedicated Direct Support professional to empower individuals with developmental disabilities to live valued lives in the community.

The position emphasizes person-centered planning, training, and ongoing coordination with families and support coordinators. You will develop, implement and monitor individualized program plans, maintain accurate records, and ensure regulatory compliance.

Qualifications

• Background and drug screenings required.
• Valid driver’s license with endorsements and 3+ years driving experience.
• Advanced computer skills required.

Responsibilities

• Develop, implement and coordinate individualized program plans.
• Develop, implement and monitor individualized training programs.
• Ensure plans are person-centered reflecting the individual’s hopes and dreams.
• Ensure documentation is accurate and compliant with regulations.
• Communicate with family, guardians and support coordinators.
