Job summary

Sunrise Community, Inc. is seeking a Direct Support professional to help individuals with intellectual and developmental disabilities live valued lives in the community.

The role emphasizes person-centered planning, training, documentation, and collaboration with families and guardians. This full-time position requires background checks, a valid driver’s license, basic computer skills, and may include supervisory responsibilities.

Qualifications

• Must pass pre-employment Background and Drug Screenings.
• Must possess a valid Driver's License with endorsements and have a minimum of 3 years of driving experience.
• High School Diploma or equivalent required; Bachelor’s Degree preferred.

Responsibilities

• Develop, implement and coordinate individualized program plans.
• Develop and monitor individualized training plans to meet identified needs.
• Ensure plans are person-centered and reflect the individual’s hopes and dreams.
• Maintain accurate and complete documentation per regulatory requirements.
• Communicate with family, guardians, and support coordinators to address needs.
• Perform other duties as assigned.
