Job summary

ChenMed in the United States is seeking a Primary Care Physician (PCP) to deliver outcomes-focused, value-based care for an aging patient panel. The role emphasizes accountability, health coaching, and actionable care plans.

You will lead a care team, collaborate with hospitalists and specialists, manage complex chronic conditions, and participate in risk and quality initiatives. State Medical License required; Board certification preferred; DEA number and BLS within 90 days.

Qualifications

• MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty.
• Must be able to obtain a State Medical License or already have a current, active State Medical License for the state(s) in which he/she will be working.
• Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred. Board Eligibility is required.
• Must have a current DEA number for schedule II-V controlled substances.
• Basic Life Support (BLS) certification from the American Heart Association or American Red Cross required within first 90 days of employment.

Responsibilities

• The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient.
• Takes full accountability for patient care and outcomes; seeks consultation when needed.
• Engages with hospitalists whenever a patient is in the hospital.
• Responsible for assessment, diagnosis, treatment, management, education, health promotion and care coordination.
• Leads their care team consisting of medical assistant, care facilitator, and care coordinator.
• Engages with transitional care teams for homebound or hospital-based patients; collaborates with case managers and other resources.
