Job summary

ChenMed, a physician-led and mission-driven primary care organization, is seeking a Primary Care Physician (PCP) to provide care for adults with chronic and acute conditions and to lead a care team. You will manage a patient panel, collaborate with hospitalists and case managers, and focus on outcomes and value-based care.

The role emphasizes accountability, health coaching, and action-oriented care, with attention to quality measures and patient satisfaction.

Qualifications

• Fluency in English, verbal and written.
• Independent judgment is required.
• Ability and willingness to travel locally as needed for market training and recruiting.
• Willingness to lead and collaborate with care teams and hospitalists as needed.
• Commitment to evidence-based medicine and continuous improvement.

Responsibilities

• Independently provides care for patients with acute and chronic illnesses.
• Take full accountability for patient care and outcomes and seek consultations when needed.
• Engage with the hospitalist whenever one of their patients is in the hospital.
• Lead their care team including MA, care facilitator, and care coordinator.
• Coordinate care for patients in hospital, SNF, LTC or homebound with case managers and other resources.
• Participate in risk and quality management programs and center leadership.
