Job summary

PACS is seeking a licensed Physical Therapist to help patients regain mobility and improve quality of life. You will evaluate and develop personalized treatment plans, provide hands-on therapy, and collaborate with physicians and the healthcare team to coordinate care.

You will supervise PTAs and students, participate in care conferences, and document progress. A strong communicator with teamwork skills shines in our fast-paced environment with ongoing learning opportunities.

Qualifications

• Licensed Physical Therapist in good standing with the state.
• Bachelor’s degree required; Master’s or Doctorate (DPT) preferred.
• Strong communication and teamwork skills.
• Comfortable using electronic medical records (e.g., Casamba, PointClickCare).
• Ability to solve problems, think critically, and adapt in a fast-paced environment.

Responsibilities

• Evaluate patients within 24 hours (or 1 business day) of referral
• Develop and implement personalized treatment plans in collaboration with physicians
• Provide hands-on therapy in alignment with approved care plans
• Monitor and document patient progress, updating plans as needed
• Educate patients, families, and caregivers for successful recovery and discharge
• Assist with discharge planning and recommend appropriate equipment (e.g., walkers, braces)
• Collaborate with nurses and healthcare team members to ensure coordinated care
• Supervise Physical Therapist Assistants (PTAs), aides, and students
• Participate in care conferences, team meetings, and ongoing staff training
