Job summary

PACS Group is seeking a licensed Physical Therapist (PT) to deliver high-quality patient care. You will evaluate patients within 24 hours of referral and develop individualized plans in collaboration with physicians, providing hands-on therapy aligned with approved care plans.

You will monitor progress, educate patients and families, and assist with discharge planning in a dynamic clinical setting. Collaboration with nurses and the healthcare team is essential, with supervision of PTAs and

Qualifications

Bachelor's degree required; licensed in good standing.
Master's or Doctorate (DPT) preferred.
Excellent communication and teamwork skills.

Responsibilities

Evaluate patients within 24 hours (or 1 business day) of referral.
Develop and implement personalized treatment plans in collaboration with physicians.
Provide hands-on therapy in alignment with approved care plans.
Monitor and document patient progress, updating plans as needed.
Educate patients, families, and caregivers for recovery and discharge.
Assist with discharge planning and recommend equipment as needed.
Collaborate with nurses and healthcare team for coordinated care.
Supervise PTAs, aides, and students.
Participate in care conferences, team meetings, and ongoing staff training.
