Job summary

Elite Hospice of Mena, part of LHC Group, is hiring a Per Diem Registered Nurse Case Manager to deliver the Plan of Care for hospice patients in Arkansas. You will evaluate performance and ensure care quality while supporting hospice policies.

Pay ranges $35.45 to $53.15 per hour, based on experience and local market. Benefits include a comprehensive package, incentive programs, and 401(k) contributions; per diem schedule offers flexibility.

Qualifications

• Current unrestricted RN licensure in the state of practice.
• 1+ years of clinical experience.
• Current driver's license, valid vehicle insurance, and access to a dependable vehicle, or public transportation.

Responsibilities

• Make the initial nursing evaluation to determine hospice eligibility within 48 hours of referral.
• Identify and reassess patient/family physical, psychosocial, and environmental needs every 15 days or as needed.
• Initiate and coordinate the plan of care with accurate clinical notes.
• Document problems, goals, interventions, and patient/family responses to hospice care.
• Collaborate with patient/family, attending physician, and IDG members daily.
