Job summary

Optum is seeking a Per Diem Registered Nurse Case Manager to assume full nursing responsibility for the Plan of Care for each hospice patient and to continually evaluate performance to improve productivity and quality of care.

Primary responsibilities include the initial nursing evaluation within 48 hours of referral, identifying patient needs, coordinating the plan of care with timely documentation, and collaborating with the patient, family, attending physician and the IDG to deliver

Qualifications

• Current unrestricted RN licensure in the state of practice.
• Minimum 1 year of clinical experience.
• Valid driver's license and access to reliable transportation (or public transit).

Responsibilities

• Initial nursing evaluation within 48 hours of referral to determine hospice eligibility.
• Identify patient/family needs and reassess at least every 15 days.
• Initiate and coordinate the plan of care with accurate clinical notes.
• Document problems, goals, interventions and responses to care.
• Collaborate with patient/family, attending physician and IDG daily.
